What is a Poker case
This keyboard indeed is not designed to replace any PCB in any existing case.

The Poker form-factor case idea is a good one. There are lots of very nice after-market Poker cases. I think I remember matt3o having the measurements in a thread from the past few months but I can't find it...

what is the exact width/height of the board and what switch spacing did you use (19mm)?

switch spacing: 1 unit = 0.75" = 19.05mm
dimensions:
5.35 units tall and 15 units wide, shifted by 0.3 units from bottom, ie 0.3 units gap to bottom edge and 0.05 units to top edge, 0 units to left and right edges.
4.0125" x 11.25" = 101.9175mm x 285.75mm

Well, the definition (according to Cherry) of 1 unit is precisely 0.75" and 19.05 mm, so I have no other choice.

they have 2 specs: EU and US. For the US one they picked the closest value to 19mm which is 0.75". In fact in the EU specs the switch hole is exactly 14mm.

Plate cut-outs are indeed 14mm but switches are 19.05mm apart!
There are not 2 specifications!

Keymodule_MX.pdf that shows all information I needed to make the PCBs, including 1 units == 19.05mm!

that seems a rather old document. Anyway, it works for me!
I have two documents which are pretty close to that one, one entirely in inches the other in mm.

No chance of this fitting in Poker cases, like Pexon's cast metal ones, I assume? The extra PCB area for the buttons doesn't look forgiving.

Re Mousewheels: I just opened a cheap Dell mouse that had an optical wheel. Very fine spokes. Model number is M-UAN DEL1.
Dell creates detents by letting a spring wire reach into the spokes. Krrrk krrrk krrrrk. When you remove it the wheel runs smoothly but stops almost immediately due to low weight. Maybe put it in a lathe, reduce the diameter and press it into a metal ring.

I want to use a Logitech G9(x) scroll wheel or even better one from a Microsoft Office Keyboard which I guess won't work with qHack.

^^^ Microsoft products are always ugly, no matter if software, or keyboards!

^^^ The reason why the qHack must be a little bit larger is because there are regular switches in the way of these 2 mouse buttons!

Hint: This is the original qHack design, that webwit asked to be made.
As you can see, there are 3 mouse buttons that are below the bottom row.
