That looks like a rebranded DEC LK401. The FCC ID on the label on the back should contain "LK401" or a similar product number. The LK501 is a variation.

They were made in many variants with application-specific labels on the keys, but I had not seen a rebranded one in different colour before. I have had a regular variant that was used with DEC's generic terminals.

Honeywell was (one of?) the OEM for the LK401. I too have never seen a rebranded one, but I'm also not surprised to discover that they exist.
