Searching around, again the only source I've seen with a similar one is clickykeyboards.
Model 1394540 Gallery
It could use a cleaning but works great.
Any information on how rare these are?

I didn't think this was quite as rare, but I didn't know what it was until an hour ago. My posting of the other two has lead me to go through my stash of M's. Looked into this one and saw the speaker, so I opened it up and took pics...
I have two more whites with a missing back label that I think are relatively early models but I've only opened up one of them.
Three standard 1391401's, and three M-13's: one black, one white, one industrial new in the box that has never been hooked up to anything.

They were keyboards from IBM's RS/6000 workstations. Less common than the PS/2s, but not exactly rare either.
